EVENT - WEBSITE - SOCIAL MEDIA INFORMATION REQUEST FORM

An Event and Information Form must be completed and returned to the church office for all events and to reserve a room, as well as any information you wish to be placed into the weekly bulletin, or on the church website or social media. These forms may be obtained in several ways: 1) call or email the church office and a form may be emailed to you, 2) pick one up from the Ministry Board located behind the Worship Center, or 3) click here if you wish to download or submit your form online.

PLEASE NOTE: In order to secure a facility request your completed form must be received and approved by the church office. Thank you for your cooperation!
Form Instructions: Please note that Landmark Church does not support the usage of any of it's facilities where money is accepted or exchanged for personal gain or benefit that does not support or enhance the overall purpose and ministry of the church. (Do not leave any space blank. Place an n/a if no answer is required).
